We are thinking about moving services from Trinity Street to Bowthorpe, and we would like to hear your views. The Trinity Street building is becoming harder to maintain. Bowthorpe has more doctors, more appointments and better facilities.

We know that travelling further could be difficult for some people, so we want to understand how this change might affect you. Please share your views by completingthe online survey.

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.

It can take a while for your medical records to reach the practice therefore the information you provide will help the clinicians should you need to be seen whilst we await your records.
