Access to records

In accordance with the Access to Health Records Act, patients may request to see their medical records. You will be required to provide 2 forms of ID as part of this application; this is in line with national best practice to ensure that your confidential health record is protected. No information will be released without the patient consent unless we are legally obliged to do so. The practice will have 30 days to provide this to you.
