Patient Participation Group registration

Use this service to join our Patient Participation Group.

The Patient Partnership Group (PPG) works with the practice to maintain and improve the service to patients.

We meet 4/5 times a year to discuss issues, concerns and opportunities which affect our community directly with GP’s and the Practice team. We believe that the meaningful participation of patients is vital to quality service provision.

Once the form is completed the practice manager will liaise with the PPG lead who will contact you.

• are registered at the surgery

• if applicable, the details of the person you are completing the form on behalf of
